When Traditional Antidepressants Fail: Is TMS or Ketamine Therapy Right for You?
When antidepressants fall short, it’s time to look beyond medication alone. Discover how TMS and ketamine infusion can help you find lasting relief from depression.

If you’ve tried antidepressant after antidepressant, but still don’t feel like yourself, don’t be discouraged. There’s still hope!

Today’s mental health care offers more than ever before. At Greathope Psychiatry and Behavioral Health Services in Garland, Texas, Chima Asikaiwe, MD, Cecilia Chanda, DNP, PMHNP, and Amy Baynash, PMHNP, provide advanced treatments like transcranial magnetic stimulation (TMS) and ketamine therapy for patients who haven’t had success with traditional antidepressants.

Both treatments have helped many people with treatment-resistant depression, but they work in different ways. Understanding those differences can help you have a more informed conversation with our team about your next steps.

Why don’t antidepressants work for everyone?

Traditional antidepressants target serotonin and norepinephrine, which are chemicals in your brain that regulate mood. While they're highly effective for many people, they don't work for everyone, and they often take several weeks before you notice improvement.

If you've tried multiple medications but still feel low, you may have treatment-resistant depression (TRD). Despite the name, that doesn't mean your depression can't be treated. It means it's time to consider a different approach.

Fortunately, treatments like TMS and ketamine therapy often work when traditional antidepressants don’t.

Understanding TMS and ketamine therapy

Although both treatments help with depression, they work differently.

TMS

TMS uses gentle magnetic pulses to stimulate areas of your brain that regulate your moods. It’s noninvasive, and you stay awake during the session. You can return to work, school, or your regular activities immediately after the appointment.

Ketamine therapy

Ketamine is a dissociative anesthetic that works by targeting your brain’s glutamate system rather than serotonin. This different approach may help create new neural connections and relieve symptoms more quickly than antidepressants or even TMS.

Here’s a quick comparison:

  • TMS is noninvasive and doesn’t require medication during treatment
  • Ketamine infusion therapy may provide faster relief
  • Both have success for TRD
  • Neither treatment is a first-line option, but both can be highly effective when traditional medications haven’t worked

Which treatment is right for you?

There's no single answer because every person's experience with depression is different. Several factors can influence which treatment we recommend, including:

  • Your medical history
  • The severity of your symptoms
  • Previous medication trials
  • Other mental health conditions
  • Your treatment goals

Some patients are better candidates for TMS, while others may benefit more from ketamine therapy. In certain situations, you may receive both treatments at different stages of your care.

That's why a comprehensive psychiatric evaluation is so important. Rather than guessing, we take the time to understand your unique situation before recommending a treatment plan.

You still have proven options

Living with depression that hasn't responded to medication compounds what you're feeling. It's easy to lose hope after trying treatment after treatment without much improvement. Remember, though, that treatment-resistant doesn't mean untreatable.

We offer medication management, talk therapy, TMS, ketamine therapy, and comprehensive psychiatric evaluations because we know that no single treatment works for everyone. Our goal is to find the combination of care that helps you feel like yourself again.

If antidepressants haven't given you the relief you deserve, it may be time to explore another method. With today's advanced treatment options, you can find hope, healing, and lasting relief.
